Advance Care Planning

Advance care planning (ACP) is a process by which patients formulate and express wishes regarding treatment to family and care providers. Despite ongoing efforts, advance care planning engagement between patients and providers is suboptimal. As in many clinical contexts, three main barriers exist in the renal setting: 1. Lack of shared understanding regarding ACP, 2. Delaying conversations, and 3. Lack of a routinized process. This talk offers a practical approach to these issues based upon findings from local and national ACP research.
